Our MISSION STATEMENT

F.A.I.T.H. Incorporated: We assist men, women, and youth in obtaining their State of Illinois ID card, birth certificate, social security card, and voter registration card which permits one to apply for employment, education, housing, and other viable needs which are provided at no charge to the clients.

 

F.A.I.T.H. Incorporated’s primary goals are to reduce recidivism, promote community safety, and help alleviate and build capacity and leadership to formerly incarcerated individuals and their families.

 

F.A.I.T.H. Incorporated continues to work and collaborate with other organizations to establish programs and services through a referral resource network designed to assist the community and meet the needs of men, women, and youth returning back to the community through the re-entry process.

   

Our Purpose

Established by Reverend John H. Crawford, Jr. in 1987, F.A.I.T.H. Inc. is a non-for-profit, grass root, self-help organization that is dedicated to providing formerly incarcerated inmates from the State of Illinois Correctional facilities with services and referrals to active social service resource agencies that are currently available and operative within the city and suburban communities. It is our intent to help these men and women with special needs services as they re-enter the community, reunite with their families, and begin to reconstruct their lives.

 

F.A.I.T.H. Inc. along with the State of Illinois Department of Corrections Community Advisory Group District One (CAG) has provided services and legislative issues on behalf of thousands of incarcerated and formerly incarcerated men and women. The results of our partnership with CAG have been rewarding and positive for all members involved. We currently have a body of members of approximately 75 various social service organizations that share their knowledge and expertise with  formerly incarcerated individuals, community members, and other existing social agencies currently active within the community. The gathering of the CAG has been a great vehicle for networking, seeking and/or gaining employment in addition to interacting with all levels of individuals involved in this type of service.

 

The extent of services that F.A.I.T.H. Inc. and the CAG are able to share with formerly incarcerated individuals is endless. Together we provide information and references for employment/training opportunities, education, housing, legal information, advocacy on prison issues and reform, medical and substance abuse counseling, clothing and food pantries, community and economic development updates, family re-integration, and spiritual guidance, etc. We acknowledge that men and women, following incarceration, will have to transition back into the community and society. These men and women are in need of many special services as they prepare to live in today’s society in hopes of regaining a constructive and positive lifestyle.

 

Once out of the institution, formerly incarcerated inmates seeking help in obtaining their personal identification can come to our organization. F.A.I.T.H. Inc. is able to assist formerly incarcerated individuals in obtaining the required identification to apply for employment, benefits, and/or education, etc.

 

The services we provide are listed below:

>> Information regarding obtainment of a Social Security Card

>> Proof of Address (Change of Address Forms)

>> School Transcript Application (CPS available at our facility)

>> Mentoring

>> Pastoral Counseling

>> Family Re-Integration

>> Referral and Networking Source (Community Advisory Group)

 

F.A.I.T.H. Incorporated, through contractual agreement(s) and federal funding, will continually look forward to assisting formerly incarcerated individuals who first and foremost are in need of special services to obtain their Illinois State Identification Card and birth certificate, which are the two main viable pieces of identification that are needed for all other services applied for and received by those who are re-entering the community.

 

We encourage men and women during their transition period back into the community to remain focused and sincere, with the intent and desire to renew their mind and not fear change—always look forward—never backwards.
